- Recent Photos: Two (2) copies 2” x 2” Black & White picture and two (2) copies 1” x 1” Colored picture
- Certified true copy of Medical Specialist Diploma and/or Certificate of an Accredited Residency Training Program (R.A. 1080)
- Certificate/Diploma from a Medical School
- Certificate/Diploma of Medical Board Examination given by PRC and Updated PRC card
- Certificate of Membership in good standing from PMA
- Membership fee of Three Thousand pesos (₱3,000.00) which can be paid in installments, plus application fee of Five Hundred pesos (₱500.00).
